A qualitative evaluation of a brief multicomponent intervention provided by lay health workers for women affected by adversity in urban Kenya

Background:
Problem Management Plus (PM+) is a brief multicomponent intervention incorporating behavioral strategies delivered by lay health workers. The effectiveness of PM+ has been evaluated in randomized controlled trials in Kenya and Pakistan. When developing interventions for large-scale implementation it is considered essential to evaluate their feasibility and acceptability in addition to their efficacy. This paper discusses a qualitative evaluation of PM+ for women affected by adversity in Kenya.
Methods:Qualitative interviews were conducted with 27 key informants from peri-urban Nairobi, Kenya, where PM+ was tested. Interview participants included six women who completed PM+, six community health volunteers (CHVs) who delivered the intervention, seven people with local decision making power, and eight project staff involved in the PM+ trial.
Results:Key informants generally noted positive experiences with PM+. Participants and CHVs reported the positive impact PM+ had made on their lives. Nonetheless, potential structural and psychological barriers to scale up were identified. The sustainability of CHVs as unsalaried, volunteer providers was mentioned by most interviewees as the main barrier to scaling up the intervention.
Conclusions:The findings across diverse stakeholders show that PM+ is largely acceptable in this Kenyan setting. The results indicated that when further implemented, PM+ could be of great value to people in communities exposed to adversities such as interpersonal violence and chronic poverty. Barriers to large-scale implementation were identified, of which the sustainability of the non-specialist health workforce was the most important one.

Processes for the identification of criminal suspects tell us a great deal about wider cultural assumptions and social prejudices regarding somatic difference; its causes, relative degree, and consequence. If early modern Europeans had something approaching a forensic science, it was astrology, which has recently garnered renewed attention from historians of ideas. Rather than assume astrology's seventeenth-century decline in the face of revolutionary natural philosophy, what follows suggests that English astrology remained significant for mundane bodily discrimination, in the context of both a more deliberate, gradual reform and the tenacity of centuries-old humoral physiology. More importantly, scrutiny of astrological practice, and the logic underpinning its lasting currency, can reveal much about the significance of bodily contrasts and the meanings ascribed to them by Tudor-Stuart folk.

The following attempts a modest reconsideration of one of the most well-known early modern Englishmen, Samuel Pepys. More precisely, the article suggests that much closer attention needs to be paid to Pepys's textuality (not sexuality). In modern times, his diary has achieved canonical status to the detriment of other texts scripted in unison with the famed Cambridge manuscript, now meticulously transcribed. Tracing the multiple textual transactions elided by the final manuscript allows for important insight into the diary's partiality. We need to resist, as much as possible, the temptation of contextualizing the diary with biographical detail because that detail is so often derived from the diary itself. We should reverse our interpretive strategy: begin with Pepys's texts (plural), unravel their main interconnections, and only then reach tentative conclusions about the man. Establishing motive is a risky business when there is no subject for interrogation. There is, however, a text that wants a more rigorous contextualization.
Evaluation of the Variety of Plasmid Profiles in S epidermidis Isolates from Hospital Patients and Staff

Plasmid profiling was used to determine the variability of normal flora isolates of Staphylococcus epidermidis in order to evaluate the usefulness of plasmid profiling for identifying pathogens. Fifteen hospital staff members and patients repeatedly had cultures taken from the hands and nares, and multiple isolates were examined for plasmid profiles. S epidermidis isolated from the nares of 15 neonates were also examined. The total number of isolates examined for plasmid profiles was 726. Repetition of profiles was common among the different isolates from a single sampling (one swab). The frequency of re-isolating similar profiles on different days varied from 7% to 13%. Simultaneous isolation of similar profiles from nares and hands on the same individual varied from 0% to 11%, the percentage being lower for personnel. Isolation of the same plasmid profile from different individuals occurred only twice and resulted in an assignment probability of Pa = 0.002 for isolates obtained from different individuals. Significantly more isolates from nares contained plasmids (97%) compared with isolates from hands (89%).Patients who had two or more isolates of coagulase-negative staphylococci with similar profiles were judged, clinically, to have infections in 12 of 13 cases. However, the likelihood of re-isolating an S epidermidis strain with a similar plasmid profile twice from the same person at different times was sufficiently high to prevent plasmid profiling from being used as an absolute criterion for infection.
